Public Records Request

You may fill out a Public Records Request Form then print and mail (PO Box 820), fax (360-568-2143), or bring the form into our Headquarter's Fire Station (1525 Avenue D). Fire District 4 has five (5) business days (excluding legal holidays) from the date that we receive the request to either answer your request or give you a reason why we cannot comply – fifteen (15) days for medical records. If the request is for medical records we will need either a copy of your identification or a signed waiver from the person involved in the incident. You will be notified upon completion of the request. At that time you can pick up a copy of the material at 1525 Avenue D during normal business hours, Monday through Friday, 8:30 a.m. to 4:30 p.m. If an email copy of information was requested, it will be forwarded to you at the address provided. Some material can be faxed if you have provided a facsimile number.
