Snohomish Fire District #4 provides a service with Vial of L.I.F.E. (Lifesaving Information For Emergencies) kits. These aid Fire District crews in gathering vital emergency information. You can come to Headquarters Fire Station at 1525 Avenue D to get your free Vial of L.I.F.E. The kits are available for anyone.

What does the Vial of L.I.F.E. Contain?

A completed Vial of L.I.F.E. form. (Answer the questions you want, don't answer questions you don't want)

Providing a picture of yourself would be helpful.

A photocopy of your last EKG will greatly assist the emergency personnel. (Your doctor should gladly provide you with a copy)

Place your Living Will or equivalent in your "vial", if you have one.

Place any Do Not Resuscitate (DNR) documentation in the pocket, if you so choose.

Place any other documentation you feel important in the pocket vial.

Attached the while sleeve prominently on a metal surface (i.e. refrigerator).
